Enrollment Requirements

Applications must include:

• A completed online application
• Non-refundable application fee
• Original copy of birth certificate
• Current record of all immunizations
• Original copy of Baptismal and First Communion certificates (if applicable)
• Questionnaire/teacher recommendation form (See instructions below.)

Have your child’s current Day Care, Preschool teacher, or teacher complete the questionnaire/teacher recommendation form. Have the school email it to admissions@ollnovato.org or mail it to:

NOTE: Your application will not be considered “complete” until all of the above-referenced information is submitted to the school office. When completed applications are received, your child will be scheduled for a Readiness assessment (TK-1st) or Shadow Day (Grades 2nd-8th).
 
Families will be notified by email of their child's acceptance within two weeks of your child’s assessment / shadow date. Once you have received notification of acceptance, you will need to send in your deposit to hold your child’s spot.  After the deposit is received, the school’s Bookkeeper will contact you to start the Registration Process.
